Prior Authorization Update for Evolent Effective 12/1/25

Evolent (formerly New Century Health) manages prior authorizations for Centene for Medical Oncology, Radiation Oncology, Pediatric and Dose Optimization, and Cardiology. Beginning on 12/1/2025 the procedure codes outlined in the accompanying document will require prior authorization through Evolent.
