NH Healthy Families Prior Authorization Update: BRCA Genetic Testing

Effective for dates of service including and after July 1, 2018, NH Healthy Families will be covering the BRCA gene testing codes listed below. Effective for dates of service on and after September 1, 2018, prior authorization will be required. Prior authorization can be requested through the Secure Portal or by completing the forms on NHhealthyfamilies.com and faxing the health plan. You can also use the Pre-Auth Check tool located on the NH Healthy Families website under Provider Resources to check authorization requirements.
