Case Management Available to Members with Special Needs

Case Management
We understand some members have special needs. In those cases, NH Healthy Families offers case management services. Case management is a service that helps members with complex medical needs get the care they need. Pregnant members, members with special health care or behavioral health needs or a disability could benefit from case management.
Conditions may include:
- Sickle Cell
- Multiple Sclerosis
- Kidney or Renal Disease
- Organ Transplants
- HIV/AIDS
- Hemophilia
- Behavioral Health
These members often see several doctors or may need medical supplies or help at home. That’s why our case managers are Registered Nurses or Social Workers, who can help arrange care with providers and help you better understand your health problems.
If you have special healthcare needs or a disability, case management may be able to help you. Member Services can give you more information. You can ask to speak to a case manager.
