NH Community Mental Health Targeted Case Management

As you are aware, NH Healthy Families performs onsite chart reviews at New Hampshire Community Mental Health Centers on a monthly basis. We would like to inform you of an additional audit we will be performing beginning July 16, 2018.

Health plan staff will continue to review a sample of your eligible members’ records as they do today. An additional audit will focus on select members’ Bureau of Behavioral Health’s eligibility for targeted case management and adherence to the He-M 400 rules as it relates to the targeted case management services.
